Document Checklist and Instructions

A properly documented loan application makes your loan process go smoothly and help lessen amount of work due to lender conditions. This checklist will help you gather your paperwork.  Please fax to 408-317-9524.

  1. Complete and sign the residential loan application, Form 1003, using one of the above options. Page 4 of the application is a continuation page in case you need additional space for your assets or liabilities. If you make a mistake while filling out the application cross it out, make a change and initial next to the change. Do NOT use whiteout.

  2. If you are salaried: provide W-2's for the previous two years and one month of paystubs.

  3. If you are self-employed, provide tax returns for the previous two years, including all schedules, and a YTD profit and loss statement. (Note: provide copies of all requested documents. Do not provide original documents.)

  4. If you own rental property, provide recent rental agreements and tax returns for the previous two years, including all schedules.

  5. Bank statements for the most recent three months, and recent statements for stock, mutual funds and IRA/401K accounts.  The more the better...

  6. Valid federal or state ID (ie.  driver license, passport)

  7. Current homeowners insurance policy, and if applicable, latest Homeowners Association statement.

  8. If you are requesting a cash out refinance, provide a letter explaining how you will use the refinance proceeds.   

  9. If applicable, provide a copy of your divorce decree and settlement agreement.

  10. If you are NOT a US citizen, provide a copy of your green card (front & back). If you are NOT a permanent resident provide a copy of your H-1 or L-1 visa.

  11. If any borrower has filed bankruptcy, provide the Discharge Notice, Filing and Schedule of Creditors.

  12. If you are applying for a home equity line of credit or loan (second loan), also include your first mortgage note. (This should be with your closing loan documents.) 

Home Valuation Code of Conduct Requirements

The HVCC (Home Valuation Code of Conduct) requires appraiser independence.  Loans with application dates of 05/01/2009 or later are required to be fuly HVCC compliant.  Appraisals must be ordered before the file is registered.  The appraisal is required to be in file for refinance loans when the file is registered.  Broker must allow sufficient time for the appraisal to return, expected in five days but does not guarantee.  Brokers are required to order the appraisal through lender website.  Broker may use borrower credit card to pay for the appraisal order.  The source of the charge will show as from LenderVend.  Appraisal cost depends on the state, in California, lender will charge $425 for residential appraisal.  Broker will be informed when appraisal has been delivered to Lender.   As a requirement of the HVCC, Lender will verify that borrower received the appraisal 4 days prior to the note date.  To avoid delays, borrower's email on the order form must be provided.
